In-home aged care is changing for the better. From 1 November 2025, the Australian Government will roll out the Support at Home program, replacing Home Care Packages (HCP) and Short-Term Restorative Care (STRC).
This new approach is all about making it easier for older Australians to live independently, with personalised support that fits your life and your home.
With over 80 years of experience and more than 12,000 care clients across Queensland, Churches of Christ is ready to help you make the move to Support at Home - with less stress and more confidence.

What’s new with Support at Home

The Support at Home program brings a fresh approach to in-home aged care, with more flexibility, better access to services and support that’s tailored to your needs. Here’s what you can expect from the new model.

Enhanced access

You’ll have better access to services, assistive products, equipment and home modifications that support safety, independence and staying connected. This means quicker approvals and more responsive support tailored to your needs and living situation.

Funding for complex needs

Expanded funding levels help people with higher care needs access more comprehensive support. This includes additional services, specialised care and tailored solutions to improve wellbeing and maintain independence at home.

Flexible assessments

You can access assessments and reassessments more easily as your needs change. This flexibility ensures your care plan stays relevant and responsive, helping you get the right support at the right time.

Restorative care pathway

You’ll have access to allied health services that focus on maintaining or improving your independence. This includes physiotherapy, occupational therapy and other supports designed to help you stay active and well at home.

Assistive technology and home modifications

Support tools and home modifications are now easier to access and more tailored to your individual needs. Based on your assessment, you’ll get faster approvals for equipment and changes that improve safety and comfort.

End-of-life support

Additional funding is available for people who choose to remain at home during the final stages of life. This ensures compassionate, personalised care that respects your wishes and supports your wellbeing and dignity.

Personalised care

Your care plan will be developed in partnership with your Churches of Christ provider. It’s based on your needs, assessment and budget, ensuring your support is tailored, flexible and aligned with what matters most to you.
