HELP AT HOME - What Home Care services are available?

Funding and support is available to individuals who meet certain criteria and are over 65, or 55 for Aboriginal and Torres Strait Islander people. We offer several support services that can keep you independent at home, for longer.

To access Home Care support, you will need to complete an Aged Care Assessment through the government, by visiting agedcare.health.gov.au or calling them on 1800 200 422. If you are eligible, and you would like to choose Churches of Christ services as part of your My Aged Care package, we can support you through the assessment process and identify the level of care available to you.

We will work with you to develop a care plan, and there are a variety of tasks we can help with at home, depending on your allowances.

• Garden or external maintenance
• Health and clinical care
• Shopping and errands
• Meal preparation

Here are the programs we can provide support with:

The Commonwealth Home Support Program (CHSP) provides government-funded entry-level support to help you stay living independently and safely at home. Our CHSP carers come to your home to help you with basic tasks, including housework, transport and personal care.

Home Care Packages (HCP) provide government-funded support for older Australians to receive regular assistance to stay living independently and safely at home. Our HCP carers and registered nurses come to your Queensland home to help you with a range of tasks, including health care, personal care, housework and transport. Home Care Packages are for those who need more frequent support than what's available through the Commonwealth Home Support Program.

Home Assist Secure provides government-funded support to help older Queenslanders with critical home maintenance needed to keep you safe at home. We come to your home to help you with things like repairs, minor modifications, safety and security upgrades.

Veterans' Home Care provides government-funded basic support to eligible veterans and their dependants so they can remain independent and living at home. To receive Veterans' Home Care (VHC) from us, you'll need to live in Queensland and hold either a Veteran White Card or Gold Card.
