5 steps to accessing a home care package

    It is inevitable that at some point in life, you will need some support to be able to continue living independently in your own home for as long as you desire.

    There are different types of Commonwealth funded care available:

    • The Commonwealth Home Support Programme (CHSP)
    • The Home Care Support Programme.

    For those with higher care needs The Home Care Package Programme provides a range of comprehensive, tailored care and services including:

    • Personal Care
    • Support services
    • Nursing, allied health and other clinical services
    • Care Coordination and case management.

    There are four levels of Home Care Packages, catering to different levels of needs, and each package is granted a subsidy, which is paid directly to your provider by the Australian Government.

    Your chosen Home Care provider will help tailor make a package that will best support your needs and goals.

    To access a Home Care Package, there are five steps involved:

    1. Check your eligibility – contact My Aged Care
    2. Find a Home Care Package provider
    3. Work out the costs
    4. Accept a Home Care Package
    5. Begin your services.
